Getting Started
Getting started with Family Preservation begins with a support request or an authorized referral. BridgePoint reviews the information to understand your needs, the requested services, and the appropriate next steps. A request does not guarantee acceptance, and formal services may require additional authorization or review.
Family Preservation services are based on each family’s needs, BridgePoint’s approved service scope, service area, staffing, and available capacity. During the review process, the team considers whether the requested support is appropriate and can be provided safely. If BridgePoint is not the right fit, staff may share approved resource-navigation information when available.
Family participation begins with clear communication, respect, and an opportunity to share your perspective. BridgePoint works to understand your family’s strengths, concerns, priorities, and goals while explaining service expectations and privacy. Participation requirements may depend on the referral, service agreement, court, or case context.
Family Preservation services are individualized and may include coaching, practical skill-building, coordination, and resource navigation. The family and service team work toward goals connected to family priorities, safety, and daily routines. Service location, frequency, and duration depend on the approved plan, authorization, family needs, and provider capacity.
BridgePoint should handle family information through appropriate, secure processes and share only the information necessary for authorized service purposes. Confidentiality may have limits based on applicable law, policy, contract, or mandatory-reporting requirements. Website forms are not emergency channels, and confidential case details should only be shared through approved secure processes.
Transition planning helps families review progress, recognize strengths, identify remaining needs, and prepare to continue helpful strategies after services end. When available and appropriate, BridgePoint may provide approved community resources, clarify follow-up responsibilities, and coordinate authorized communication with referral sources. Service completion does not guarantee that every family goal or external case requirement has been met.
